When reconstructing the maxilla due to monolateral or bilateral atrophy in partly or totally edentulous patients, there are some alternative treatments that should be taken into consideration. Insufficient bone volume in the posterior maxilla is not in itself an indication for performing a lateral window maxillary sinus augmentation without considering available alternatives.

Careful evaluation of the possible alternatives is mandatory for all patients who, due to general health conditions, pathologies of the maxillary sinus, advanced age or psychological reasons cannot undergo invasive surgery.

With regards to the clinical condition, there are different treatments that have been proposed as alternatives to lateral window sinus augmentation:

  • short implants
  • tilted implants and trans-sinus implants
  • pre- and post-sinus implants
  • fixed prostheses with distal cantilever

The lecture will address the pros and cons of the different therapeutic alternatives in an evidence based.

The esthetic implant rehabilitation of patients with esthetically and functionally compromised dentition frequently involves a multidisciplinary approach and represents a clinical challenge.

During recent years, treatment modalities and options have changed and from a surgical point of view, less invasive procedures are used in order to preserve soft tissue anatomy and minimize recession. Also the design of implants and prosthetic components have changed in order to minimize bone resorption and soft tissue alterations.

This presentation will give practical guidelines that can be easily used in day-to-day clinical practice in order to help maximize peri-implant esthetics. Basic soft and hard tissue reconstruction procedures will also be discussed to address pre-existing deficiencies. Treatment algorithms will be outlined to help guide the clinician to choose the various procedures and techniques to predictably meet patient expectations.
